Management of Hepatic Metastases from Colorectal Cancer

Abstract
The present report analyzes the 105 cases who had hepatic metastases from the colorectal cancer. Of them, 80 cases, which were 10.2% of total patients with primary colorectal cancer who was admitted to SNUH from Jan. 1973 through Dec. 1983, had synchronous hepatic metastases. The percentage of hepatic metastases from right colon, left colon, and rectum were S%, 27.6%, and 10%, respectively. In 29. 5% of patients the right lobe was involved, and the left lobe only involved in 10% of patients. The majority of patients had metastases to both lobes. Solitary metastases was recorded in 31. i% of patient, and the multiple metastases to one or both lobes. And 21% of the metastatic cases were considered to be resectable. The liver scan correctly predicted the metastases in 57.5% of patients, and CEA levels were elevated in 77% of patients with hepatic metastases. Twenty five point five percent: and 12.5% of patients with hepatic metastases had elevated alkaline phosphatase level and GOT/GPT level, respectively. Additional sites of metastases besides the liver were recorded in 22 cases; Lung 7.6%, Extraabdorainal lymph node 7.6% and Bone 4.8%. Among those patients who underwent resection of primary lesion as a palliative measure, the median survival time was about 7 months, while those patients with hepatic metatases treated by palliative diversion survived for a median time of 5 months. The statically significant difference between the survival time of patients with single hepatic metastases and that of patients with multiple metastases in both lobes was not present in this report, but there was inverse relationship between the extent of hepatic involvement and the duration of survival. The difference between the survival time of the untreated group and that of patients who was treated with chemotherapy after surgical treatment of primary site was present, but the difference was not statically significant.
